GUIDELINES
You may qualify for assistance through Dental OPTIONS if you do not have dental insurance and your household is below 200% of the Federal Poverty Guideline ($1,634 gross monthly income for household of 1, $2,200 gross monthly income for household of 2, $3,334 gross monthly income for household of 4).
Completely free treatment may be available if you do not have dental insurance, your income is below 100% of the Federal Poverty Guideline AND you are a senior or are mentally, medically or physically challenged.
RULES
After you are accepted in the Dental OPTIONS Program you must:
- Call and make your appointment within one week of contact with your Dental OPTIONS Referral Coordinator.
- Call the dental office 24 hours before your appointment if you need to cancel your examination. If you do not show up for your appointment and have not called, your dentist may choose not to do your dental work.
- Be on time for your appointment.
- Pay the discounted fee at the time services are provided.
- Be responsible for your transportation to and from the dental office.
- Keep your Referral Coordinator informed of your treatment progress.
Please note:
- Your dentist has volunteered to complete only the needed dental work that you have agreed to.
- The Dental OPTIONS Program does not require the dentist to keep you as a patient after your dental work is completed.
- If you do not show up for your appointment and do not cancel your exam, the dentist may choose not to do your dental work. If you miss two appointments, you will be removed from the program.
